  • The information you provide in this form may be used for Advocacy purposes to assist customers across fines and fees in Revenue NSW.
  • You have read the requirements and agree to the Advocate Charter.
  • You have and will retain a written Authority to Act from your customer.
  • You have read and agree to the Privacy Collection Notice.

Revenue NSW is collecting your personal information to assess your eligibility to be an Advocate for our Fines and State Debt customers. Revenue NSW gives priority to protecting the privacy of your personal information. We do this by handling personal information in a responsible manner and in accordance with the NSW Privacy Laws: Privacy and Personal Information Protection Act 1998 and Health Records and Information Privacy Act 2002.

We will collect the following information from you:

  • Your name, email address and contact preferences.
  • Your organisation name, address and position held.
  • Evidence to support your authority/position within an organisation.
  • Evidence our customer has given you authority to act on their behalf.

Revenue NSW may use your information to:

  • Contact your organisation to confirm your position in the organisation.
  • Respond to complaints and enquiries including disclosure to relevant authorities such as NSW Ombudsman, the NSW Civil and Administrative Tribunal.

Revenue NSW may disclose this information to:

  • Your organisation, for the purpose of managing your authority to be included on the register and for audit purposes.
  • The authority who has issued a fine or fee such as NSW Police, Local Councils or Transport for NSW or as permitted by law.
  • The customers you are representing as an Advocate.
  • Other third-party service provider engaged by Revenue NSW to support the advocacy program.

Providing us with the requested information is not required by law. However, if you choose not to provide us with the requested information, we may not be able to approve you as an Advocate to act on behalf of Revenue NSW Fines and State Debt customers.

Revenue NSW will take reasonable security measures to protect your personal information from loss, unauthorised access, use, modification, disclosure, or other misuse. Your personal information will be held and disposed of securely.

You have a legal right to access and update your personal information.
